Job Description

Mission Hospital, located in Asheville, North Carolina, is a flagship healthcare facility and serves as the region's referral center for various critical healthcare services. The hospital is licensed for 815 beds and has received numerous awards for clinical excellence and safety. As a Registered Nurse in Case Management, you will work collaboratively with the healthcare team to provide comprehensive patient care and ensure effective discharge planning while utilizing the hospital's documentation systems, including CERNER, NATE, and ENSOCARE.

Responsibilities

• Collaborate with healthcare team members to facilitate effective patient care transitions.
• Conduct assessments and manage patient care plans.
• Utilize documentation systems accurately for patient records.
• Build trust with patients and their families while providing education and support.

Required Experience

• Demonstrated ability to work collaboratively and communicate effectively in a healthcare setting.
• Previous experience in case management or a related area of nursing is preferred.

Certifications/Licensure

• Current RN license in North Carolina.
• Relevant certifications in case management are preferred but not required.
