Job Description

As a Registered Nurse in Case Management at Mission Hospital, you will work collaboratively with the medical team to ensure effective patient care management. Your responsibilities will include comprehensive assessments, coordination of services, and discharge planning for patients, ensuring high-quality outcomes in a supportive healthcare environment.

Responsibilities

• Work collaboratively with RN and MD to achieve optimal patient care.
• Conduct patient assessments and develop individualized care plans.
• Coordinate with healthcare team members to ensure comprehensive patient support.
• Document patient care activities accurately and timely.
• Build trust and rapport with patients and their families.

Required Experience

Must possess strong documentation and communication skills, along with the ability to shift attention between tasks efficiently.

Certifications/Licensure

Active RN license is required. Certification in Case Management is preferred.
