Job Description

The Physical Therapist will be responsible for evaluating and treating patients with physical impairments, disabilities, and other limitations. This role will focus on improving patients' mobility, strength, and overall quality of life while working collaboratively with the healthcare team in a post-acute care setting.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and needs.
• Develop and implement personalized treatment plans to address patient goals.
• Utilize therapeutic techniques to enhance physical function and mobility.
• Educate patients and families on exercises and techniques for continued improvement.
• Document patient progress and communicate with the healthcare team regularly.

Required Experience

Must possess a valid Physical Therapy licensure and have experience in a post-acute care or rehabilitation setting.

Certifications/Licensure

Current state licensure as a Physical Therapist is required.
