Required to work every other weekend. Day Shift, 5x8 hours (08:00 - 16:30).

Job Description

The Registered Nurse in Case Management will be responsible for care coordination, discharge planning, and ensuring compliance with CMS standards. Experience with HMOs, IPAs, and handling protective services process is required. The ideal candidate will have a strong focus on patient advocacy and resource management within an acute care setting, utilizing the Epic charting system.

Responsibilities

• Coordinate patient care and discharge planning.
• Manage appeals and denials process.
• Evaluate patient needs and develop care plans.
• Adhere to HIPAA guidelines and CMS standards.
• Support interdisciplinary team collaboration.

Required Experience

• Minimum of 2 years of acute care experience required.
• Experience in case management is a must.
• Previous charge experience preferred.
• Must be knowledgeable about DRG (Diagnosis Related Groups) and utilization review.

Certifications/Licensure

• Valid RN license required (compact license must be in hand at submission).
• BLS certification preferred.
