Job Description

We are seeking an experienced RN Case Manager to join our team on the 5th Floor Neuro Step Down unit. The position involves managing patient care and coordinating services to ensure optimal patient outcomes. The unit has a capacity of 40 beds and typically requires collaboration between case managers and social workers.

Responsibilities

• Coordinate patient care and services within the healthcare team.
• Conduct assessments and develop care plans for each patient.
• Ensure compliance with hospital policies and regulations.
• Collaborate with medical professionals, patients, and families to optimize care delivery.

Required Experience

• Minimum of 1 year of acute care case management experience.

Certifications / Licensure

• Current RN license.
