5 days a week, 8-hour shifts from 7:00 AM to 3:00 PM.
Job Description
The Licensed Vocational Nurse (LVN) will provide routine home health visits within the coverage area of Lower Brazoria County, which may expand to a 60-mile radius from the facility's address as needed. The role requires managing care for patients and ensuring compliance with health care protocols.
Responsibilities
- Perform routine home health visits as required.
- Document patient care and maintain accurate health records.
- Collaborate with healthcare teams to provide optimum patient care.
- Ensure compliance with health care protocols and standards.
- Complete 32 points of visits per week.
- May assist in other territories within the facility's service area.
Required Experience
- Recent home health experience is required.
- Experience with HomeCare HomeBase (HCHB) is preferred, but candidates without this experience may be considered at a reduced rate for training.
Certifications/Licensure
- Current LVN license is required.
