Physical Therapist - Outpatient Care

Job Description

This position involves providing outpatient physical therapy services, where the therapist will evaluate and develop individualized treatment plans for assigned patients. The focus is on effective, comprehensive care to facilitate patients' return to community activities. The physical therapist will ensure delivery of high-quality services in accordance with professional standards and organizational values.

Responsibilities

• Conduct comprehensive evaluations of patients and develop appropriate treatment plans and goals.
• Provide direct, one-on-one patient care, ensuring effective progression towards discharge.
• Document patient care in compliance with clinic policies and regulatory guidelines.
• Ensure delivery of care under signed physician orders and maintain compliance with insurance approvals.
• Act as an educator for patients, families, students, and coworkers.
• Participate in student training programs, providing supervision and evaluations.
• Engage in continuous professional development and pursue continuing education opportunities.
• Maintain professional licensure and adhere to ethical standards and codes of conduct.
• Promote a culture of safety for patients and employees while adhering to safety policies and procedures.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification.
• Active physical therapy license in the state of practice.
• Previous experience with the agency is highly desired.

Schedule

This role operates on a 5x8.0 schedule with shifts commencing on December 23rd, covering a contract duration of 6 months.
