Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans according to the patients' needs, goals, and physician direction. This role involves delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals that promote patient progress toward discharge.
• Document medical records clearly, concisely, accurately, and in a timely manner.
• Provide education to patients, families, and students while fostering a positive learning environment.
• Maintain compliance with state practice acts and other regulatory agencies.
• Participate in performance improvement initiatives and action plans.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for physical therapy.
