Licensed Vocational Nurse - Skilled Nursing Day Shift

Job Description

The Licensed Vocational Nurse (LVN) will assess, plan, and implement nursing care for residents in a skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing. The LVN is responsible for administering medications accurately and ensuring continuity of care between shifts by supervising the care provided by CNAs and other supportive staff members while maintaining the facility’s mission.

Responsibilities

• Assess resident conditions and nursing needs, prescribing appropriate nursing actions.
• Utilize and educate residents and families on pain management techniques.
• Document changes in residents' conditions and implement care plans.
• Administer medications following the eight medication rights.
• Notify physicians of changes in resident conditions and follow up as necessary.
• Provide thorough reports during shift changes, including walking rounds.
• Supervise other healthcare team members as needed.
• Engage residents and families in care planning and teaching.
• Participate in the orientation and evaluation of new personnel.
• Adhere to infection control procedures.
• Remain calm during emergencies and stressful situations.

Required Experience / Certifications / Licensure

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse license.
• Current BLS certification.
• Basic computer knowledge; experience with Electronic Health Records preferred.
• Ability to read and communicate effectively in English; additional languages preferred.
