Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for their assigned patients. This role includes designing functional treatment programs to facilitate effective returns to community activities and serving as an educator for patients, families, students, and coworkers. Additionally, the therapist upholds compliance with the organizational Code of Conduct and ensures an atmosphere of safety and professionalism in all interactions.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop treatment plans and specific goals aimed at patient progress towards discharge.
• Document patient care in a clear, concise, and accurate manner, adhering to state, federal, and clinic policies.
• Participate in student programs, providing supervision and evaluations as necessary.
• Create and pursue an individualized professional development plan with clinical management.
• Maintain professional licensure and adhere to continuing education requirements.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Six months to one year of outpatient clinical experience.
• Current CPR certification.
• Active physical therapy license in the state where services will be provided.

Schedule

This position follows a 5x8.0 shift schedule.
