Licensed Practical Nurse (LPN) - Full Time Day/Night 5x8 Shifts

Full Time, rotating between days and nights, 5 days a week with every other weekend and holidays.

Job Description

The Licensed Practical Nurse (LPN) will provide age-appropriate care for patients, demonstrating knowledge and skills necessary to assess and interpret patient data. The LPN works under the supervision of a Registered Nurse (RN) and collaborates with other healthcare team members.

Responsibilities

• Collect and monitor data related to residents' physical and psychosocial conditions.
• Create and contribute to culturally relevant and age-appropriate nursing care plans.
• Reassess and recommend updates to care plans based on patient outcomes.
• Maintain ongoing communication with residents, their families, and the interdisciplinary team.
• Participate in educating and mentoring staff members.
• Ensure compliance with safety protocols and clinical practices while considering cost and resource utilization.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure.
• BLS certification (AHA or RQI).
• Minimum of 1 year of experience as an LPN in a long-term care facility.
