Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing high-quality direct and indirect nursing care to residents, ensuring that the unique needs of adult and geriatric patients are met. The LPN will work under the supervision of a Registered Nurse (RN) and maintain collaborative relationships with other healthcare professionals to promote a healthy and safe environment.

Responsibilities

• Collect and communicate data relevant to residents’ physical and psychosocial conditions.
• Contribute to the development of culturally relevant care plans, ensuring involvement of residents and their families.
• Evaluate resident outcomes and recommend care plan revisions as needed.
• Maintain current knowledge and competency in nursing practice and participate in staff mentoring and orientation.
• Provide leadership within the nursing practice setting while considering safety, cost, and impact in care delivery.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure
• BLS Certification (AHA or RQI certification required)
• Minimum of 1 year of experience as an LPN in a Long-Term Care (LTC) facility

Schedule

This position offers a full-time variable shift, working 5x8 hours per week with every other weekend and holiday requirements.
