Job Description

The Licensed Practical Nurse (LPN) provides both direct and indirect nursing care to residents, adhering to the established policies and procedures under the supervision of a Registered Nurse (RN). The LPN plays a vital role in the interdisciplinary team, ensuring quality care based on the age-specific needs of the adult and geriatric populations served.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and team members.
• Contribute to the development of culturally relevant plans of care, reflecting nursing diagnoses and interventions.
• Assist in evaluating resident outcomes and recommend necessary revisions to care plans, ensuring comprehensive care.
• Maintain current knowledge and competency in nursing practice and participate in mentoring/orientation of new staff.
• Work collaboratively to promote a healthy and safe environment.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure
• BLS certification (AHA or RQI only)
• Minimum of 1 year experience as an LPN in a long-term care facility
