5x8.0 schedule, with 60-minute evaluations and 30-minute follow-up sessions.

Job Description

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans based on patient needs, goals, and physician directives. This role includes directing therapy services and ensuring the design of functional treatment programs for effective community activity return. The therapist will also serve as an educator to patients, families, students, and coworkers while adhering to professional standards and the organization's mission and core values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans to facilitate progress towards discharge.
• Document patient care procedures clearly and accurately, ensuring compliance with all regulatory guidelines.
• Provide supervision and mentorship in the student program, fostering a positive learning environment.
• Create a personal professional development plan and pursue ongoing education and professional activities.
• Maintain professional licensure and adhere to the discipline-specific State Practice Acts.
• Participate in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification is required.
• Maintains valid licensure and meets state-mandated continuing education requirements.
