Part-Time Licensed Practical Nurse (LPN)

This is a 13-week contract position.

Schedule

Shifts are rotational, requiring weekend and holiday availability. Orientation starts on a Tuesday or Thursday for a full day.

Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care to assigned residents, following established policies and procedures under the supervision of a Registered Nurse (RN). The LPN must possess a strong understanding of patient care across the adult and geriatric populations, ensuring to meet age-specific needs.

Responsibilities

• Collect ongoing data regarding resident's physical and psychosocial conditions, communicating findings to the RN and interdisciplinary team.
• Contribute to the development of age-appropriate and culturally relevant care plans that reflect nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluation of resident outcomes and ensure all aspects of care are addressed.
• Maintain current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
• Consider factors related to safety, cost, and impact when planning and delivering care.
• Provide leadership under the direction of the RN, ensuring compliance with safety standards regarding occupational exposure to bloodborne pathogens.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (American Heart Association or RQI)
• Minimum of 1 year of experience serving as an LPN in a long-term care facility
