LPN - Full Time Day/Night Rotation (5x8)

Must be able to start on a Tuesday/Thursday for full-shift orientation. This is a 13-week contract position with Day/Night rotations (5x8s).

Job Description

The Licensed Practical Nurse (LPN) must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The individual must be knowledgeable of the principles of growth and development and able to assess data reflective of the patient’s status, interpreting the appropriate information needed to identify each patient's requirements relative to their age-specific needs.

Responsibilities

• Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
• Recommends revisions to the plan of care after evaluation of resident outcomes, indicating the need for referrals to ensure all aspects of the resident's care have been addressed.
• Maintain current knowledge and competency in Nursing Practice; participate in mentoring/orientation of staff.
• Consider factors related to safety, cost, and impact in planning the delivery of care.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI)
• 1 year of experience as an LPN in a Long-Term Care (LTC) facility
