Job Description

The Licensed Practical Nurse (LPN) provides direct and indirect skilled nursing care for assigned residents, demonstrating the knowledge and skills necessary to meet the care needs of adult and geriatric patient groups. The LPN collaborates with a Registered Nurse and other healthcare team members to deliver safe and effective patient care.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions.
• Collaborate with residents, families, and the Interdisciplinary Team to develop and implement age-appropriate and culturally relevant care plans.
• Evaluate resident outcomes and recommend necessary revisions to care plans.
• Maintain current knowledge and competency in nursing practices and mentor new staff.
• Adhere to safety protocols and handle occupational exposure to blood-borne pathogens responsibly.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI only)
• At least 1 year of experience serving in an LPN capacity, preferably in a long-term care setting.
