LPN - Part Time Nights - Contract Position

Schedule

This position requires part-time night shifts, consisting of 2 shifts of 8 hours one week and 3 shifts of 8 hours the following week, including every other weekend (Friday and Saturday night) and holiday shifts.

Job Description

The LPN will be responsible for providing both direct and indirect skilled nursing care to residents, following established policies and procedures under the supervision of a Registered Nurse. The role requires knowledge and skills necessary to cater to the specific needs of patients across their life spans, especially focusing on adult and geriatric groups.

Responsibilities

• Collect data relevant to the physical and psychosocial condition of residents on an ongoing basis and communicate findings to the RN and Interdisciplinary Team.
• Contribute to the development of age-appropriate and culturally relevant care plans, reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
• Recommend revisions to care plans and coordinate with residents, families, and the Interdisciplinary Team to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice and participate in staff mentoring and orientation.
• Provide leadership in the professional practice setting under the direction of an RN, considering safety, cost, and impact in care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI only accepted)
• Minimum of 1 year of experience as an LPN in a long-term care facility
