AM shifts, Monday to Friday, 5 × 8 hour shifts, guaranteed 40 hours per week.

Job Description

The Home Health Physical Therapist will be responsible for delivering quality physical therapy services to patients in the home health setting. The role requires effective evaluation and treatment planning to enhance the patient's quality of life while ensuring compliance with home health regulations.

Responsibilities

• Conduct patient evaluations and develop personalized treatment plans.
• Implement therapy interventions and assess progress throughout the care process.
• Complete required documentation, including OASIS assessments.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Participate in weekly case conferences and provide updates on patient progress.

Required Experience / Certifications / Licensure

• Home health and OASIS experience required.
• Homecare Homebase documentation experience is required; however, the client may consider candidates without this experience at a reduced rate for the initial training period.
• Valid Physical Therapy license.
