Job Description

The Occupational Therapist is an integral member of the interdisciplinary team, responsible for developing, administering, and implementing therapeutic services designed to help patients regain their optimal level of function in meaningful daily activities, including self-care, leisure, and work. This role is under the supervision of the Home Health and Hospice Manager.

Responsibilities

• Evaluate patients' functional status and implement the physician’s plan of care.
• Complete assessments and make decisions based on objective tests and measurements.
• Communicate observations and treatment suggestions as a team member to ensure a coordinated plan of care.
• Provide education and direction to patients and coworkers as appropriate.
• Work independently and cooperatively, recognizing and protecting patient rights.
• Convey empathy and compassion to those experiencing grief, pain, or trauma.
• Document all communications, coordination, and treatment activities accurately and concisely.
• Adapt to emergency situations and remain alert to changes in patient conditions.

Required Experience / Certifications / Licensure

• Graduate of an accredited occupational therapy curriculum.
• Licensed by the State of Oregon as an Occupational Therapist.
• Valid driver’s license and proof of auto insurance.
• Current certification in BLS.
• Registration as an Occupational Therapist with the American Occupational Therapy Association is preferred.
• Evidence of continuing education and current knowledge of occupational therapy practices is desired.
