Job Description

We are seeking a licensed Speech-Language Pathologist (SLP) with over 1 year of experience in a Skilled Nursing Facility (SNF) setting. The ideal candidate will have a thorough knowledge of Rehab Optima (Net Health) and hold an active California license.

Responsibilities

• Conduct evaluations and assessments of patients' speech and language capabilities.
• Develop and implement personalized treatment plans.
• Provide ongoing therapy interventions to improve patients' communication skills.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient progress and adjust treatment plans as necessary.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in a Skilled Nursing Facility (SNF).
• Active California license as a Speech-Language Pathologist.
• Familiarity with Rehab Optima (Net Health) is preferred.
