Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Occupational Therapist (OT) to provide skilled therapy services to patients in their homes. The successful candidate will work with individual patients to improve their functionality and quality of life through tailored treatment plans.

Responsibilities

• Evaluate patient needs and establish treatment goals
• Implement therapy interventions and monitor progress
• May visit up to 6 patients per day, ensuring a focus on productivity
• Maintain accurate and timely documentation using EPIC Remote Client and Oasis
• Coordinate with interdisciplinary teams to enhance patient outcomes

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience
• Current Oregon Occupational Therapist license required
• BCLS certification
• Oasis experience is required
• Reliable transportation is necessary for patient visits
