Schedule

Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM). Note that this facility cannot accommodate 4x10 or 3x12 schedules.

Job Description

We are seeking a dedicated Home Health Occupational Therapist to provide exceptional care to adult and geriatric patients in the West Portland area. The therapist will be responsible for evaluating patient needs, creating treatment plans, and implementing therapy interventions to improve patient outcomes. The position involves visiting up to 6 patients per day with a 1:1 patient ratio and requires the ability to drive between patient locations within a maximum radius of approximately 60 miles.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Conduct therapy sessions to assist patients in achieving their therapeutic goals.
• Document patient progress using the EPIC Remote Client and Oasis.
• Educate patients and their families on treatment plans and home care.
• Maintain compliance with facility policies and standards of care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current OR Occupational Therapy license required.
• Basic Cardiac Life Support (BCLS) certification required.
• Experience with Oasis documentation is required.
• Reliable transportation is a must as travel between patient visits is necessary.
