Schedule

Day shift, 5 days a week, from 08:00 AM to 04:30 PM. This facility cannot accommodate 4x10 or 3x12 schedules.

Job Description

We are seeking a dedicated Home Health Certified Occupational Therapy Assistant (COTA) to provide exceptional care in the comfort of patients' homes. The ideal candidate will have experience in skilled home health and will be responsible for evaluating, planning, and implementing therapy interventions tailored to individual patient needs.

Responsibilities

• Provide direct patient care in a home health setting.
• Implement treatment plans as directed by supervising occupational therapists.
• Conduct patient assessments and document progress using electronic health records (EPIC Remote Client).
• Collaborate with interdisciplinary teams to enhance patient outcomes.
• Visit up to 6 patients per day while maintaining productivity standards.
• Educate patients and families on therapy techniques and home exercise programs.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current Oregon COTA license required.
• BCLS certification is mandatory.
• Reliable transportation is necessary due to travel between patient homes.
