Monday-Friday, 5 shifts per week, 8 hours per shift.

Job Description

We are currently seeking a Home Health Physical Therapist. The ideal candidate will provide high-quality, patient-centered therapy services in home settings. Responsibilities may include evaluations, treatment plan development, and implementation of therapy interventions.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Implement therapy interventions and monitor patient progress.
• Document patient care and progress accurately in Homecare Homebase.
• Participate in case conferences and collaborate with interdisciplinary teams.
• Evaluate and adapt treatment plans based on patient needs and outcomes.

Required Experience / Certifications / Licensure

• Active Physical Therapist license.
• Experience in home health and OASIS documentation is required.
• Homecare Homebase documentation experience preferred; candidate may be considered for reduced rate training for the first two weeks.
