Job Description

The Home Health Physical Therapist will provide high-quality physical therapy services to patients in their homes. This position requires the ability to perform evaluations, develop treatment plans, and implement therapy interventions tailored to each patient's needs.

Responsibilities

• Conduct patient evaluations and assessments.
• Develop individualized treatment plans based on patient needs.
• Implement therapy interventions and monitor patient progress.
• Coordinate care with interdisciplinary team members.

Required Experience / Certifications / Licensure

• Must have experience with OASIS and Epic.
• At least 1 year of experience in home health required.
• Active Physical Therapist license in the state of operation.
