Day Shift: 5x8-Hour (08:00 - 16:30). Required to work every other weekend.

Job Description

We are seeking an experienced Case Management Registered Nurse to join our team in Eureka, California. The ideal candidate must possess acute care experience and be ready to provide exceptional patient care while managing cases efficiently.

Responsibilities

• Oversee and coordinate patient care from admission through discharge.
• Assess patients’ needs and develop care plans in collaboration with the healthcare team.
• Conduct discharge planning and follow-ups to ensure continuity of care.
• Manage benefits eligibility and assist with insurance appeals and denials.
• Adhere to HIPAA regulations and maintain patient confidentiality.
• Monitor and document patient progress while coordinating services with interdisciplinary teams.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care experience.
• Experience with case management, including working with HMOs and IPAs.
• Current RN license in the state of California.
• Familiarity with Epic charting system is required.
