Outpatient Care Provider

Assignment Duration

This is a 26-week contract position.

Schedule

Shifts are from 7:30 AM to 6:00 PM, primarily on weekdays, with occasional Saturday coverage (typically 4-5 Saturdays over the contract duration).

Job Description

The Outpatient Care Provider will primarily focus on delivering high-quality care in an outpatient setting. Applicants with Epic experience are preferred. This role involves working closely with patients to ensure their needs are met in a timely and professional manner.

Responsibilities

• Provide comprehensive outpatient care to patients.
• Maintain accurate patient records and documentation.
• Collaborate with the healthcare team to ensure optimal patient outcomes.
• Participate in patient education and community outreach efforts.

Required Experience / Certifications / Licensure

• Current Oregon License required.
• BLS certification required.
• Experience with Epic preferred.
• Completion of Trinity 10 Panel Drug Screen and other health requirements prior to onboarding.
