Physical Therapist - Day Shift, 5x8.0

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patient's needs and physician direction. Responsibilities include the delegation and direction of all discipline-specific therapy services, ensuring the design of functional treatment programs for effective return to community activities. This role involves educating patients, families, students, and colleagues, while upholding the organization's standards of compliance and professionalism.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Collaborate with the interdisciplinary team to address barriers to discharge.
• Manage individual, concurrent, and group treatment sessions based on patient needs.
• Document medical records in a clear, concise, and timely manner.
• Participate in student program activities and provide supervision to students.
• Create an individualized professional development plan.
• Adhere to applicable State Practice Acts and maintain professional licensure.
• Engage in patient and professional advocacy efforts.
• Demonstrate awareness of performance improvement initiatives within the organization.

Required Experience / Certifications / Licensure

• BLS Certification required prior to the first shift.
• Valid, current State license as a Physical Therapist.
• Minimum of 2 years of experience in the field.

This position operates on a Day Shift, following a 5x8.0 schedule.
