Physical Therapist - 5x8 Day Shift

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services, ensuring the design of functional treatment programs that allow for effective return to community activities.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals focused on progressing patients towards discharge within an established length of stay.
• Collaborate with the interdisciplinary team to address all barriers to discharge, including timely equipment trialing and ordering, completion of patient and family training, and planning for continuing services.
• Effectively manage individual, concurrent, and group treatment sessions based on patient care needs.
• Complete medical record documentation in a clear, concise, accurate, and timely manner in compliance with state, federal, and organizational guidelines.
• Participate in the student program as assigned, providing a positive learning environment with appropriate supervision.
• Actively create a professional development plan and pursue continuous education and involvement in professional activities.
• Demonstrate ethical behavior in accordance with the discipline-specific professional code of conduct, maintaining licensure in good standing.

Required Experience / Certifications / Licensure

• BLS Certification required prior to the first shift.
• Valid, current State license is required.
• Minimum of 2 years of experience as a Physical Therapist.

Schedule

This position follows a daytime schedule of 5x8 hour shifts.
