Physical Therapist - Rehabilitation

Job Description

We are seeking a dedicated Physical Therapist to effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care. You will be responsible for developing appropriate plans of care following all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practices, you will support clinical interventions and administer various procedures as part of the rehabilitation plan.

Responsibilities

• Assess patient progress and adjust treatments accordingly in clinical records.
• Evaluate treatment outcomes and modify services based on patient evaluation.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate effectively with supervisors and other health team members regarding patient progress and treatment plans.
• Document all required information in the electronic documentation system as per company policy.
• Participate in required meetings and trainings including patient care conferences and rehabilitation meetings.
• Supervise and support physical therapist assistants, rehabilitation aides, and students.
• Adhere to APTA and state-required practice acts, code of conduct, and ethics during practice.

Required Experience / Certifications / Licensure

• Minimum of 1 year of PT experience; new graduates accepted if they have Level II experience.
• LTC or Rehab experience required.
• Oklahoma State License required.

Schedule

This is a days shift of 8 hours.
