Job Description

The Physical Therapist is responsible for providing direct patient care, evaluations, consultations, and treatments to assist patients in reaching maximum physical performance. This role involves performing individualized evaluations and designing goal-directed custom plans to improve patients' physical function and mobility.

Responsibilities

• Provide direct therapy and rehabilitative care tailored to individual patient needs.
• Conduct comprehensive patient evaluations to assess functional abilities and mobility.
• Develop and implement personalized treatment plans aimed at improving physical performance.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with healthcare teams to ensure cohesive patient care.

Required Experience / Certifications / Licensure

• Active Physical Therapist license in the relevant state.
• Experience in providing therapy in a clinical or outpatient setting preferred.
