Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician direction. This role includes delegating and directing discipline-specific therapy services and ensuring the design of functional treatment programs that facilitate effective community reintegration. The therapist acts as an educator for patients, families, students, and coworkers while adhering to organizational standards and promoting safety for both patients and employees.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and create treatment plans focused on achieving discharge goals.
• Document medical records accurately and in compliance with state and federal guidelines.
• Maintain professional licensure and complete all state-mandated continuing education requirements.
• Participate in a student program, providing a supportive learning environment and participating in student evaluations.
• Engage in continuous professional development and apply new knowledge to improve patient care and organizational objectives.
• Follow safety protocols and guidelines to ensure a secure environment for patients and staff.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience is preferred.
• Current CPR certification.
• Active applicable license for the state where the work will be performed.
• Previous experience with the agency is highly desired.
