Licensed Practical Nurse (LPN) - Full Time Nights

Full-Time Nights (11 PM - 7:30 AM) - 5x8s, Every Other Weekend, Holiday coverage required.

Job Description

The Licensed Practical Nurse (LPN) must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. This role requires the ability to assess and interpret patient data reflective of their status, catering especially to the needs of adult and geriatric patient groups.

Responsibilities

• Collect ongoing data regarding residents' physical and psychosocial conditions; communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.
• Contribute to the creation of an age-appropriate and culturally relevant care plan that reflects nursing diagnoses and interventions to achieve desired outcomes.
• Recommend revisions to care plans based on evaluations of resident outcomes, ensuring all aspects of care have been addressed.
• Collaborate with the resident/family and members of the Interdisciplinary Team while promoting customer satisfaction.
• Maintain current knowledge and competency in nursing practice and participate in staff mentorship and orientation.
• Adhere to defined policies and procedures under the supervision of a Registered Nurse.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• Basic Life Support (BLS) certification
• Minimum of 1 year of experience as a Licensed Practical Nurse, preferably in a Long-Term Care Facility
