Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsible for delegation and direction of all discipline-specific therapy services for those assigned patients, the therapist ensures the design of functional treatment programs allowing for an effective return to community activities.

This role also involves serving as an educator for patients, families, students, coworkers, and oneself, while performing other professional duties associated with patient care, the development of discipline-specific therapy services, and the design of program delivery. The therapist demonstrates behaviors and attitudes consistent with the organization’s Mission Statement and Core Values, upholding compliance expectations outlined in the Code of Conduct.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge.
• Maintain clear, concise, accurate, and timely medical record documentation.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student programs as appropriate, providing a positive learning environment for students.
• Actively create an individualized professional development plan with clinical management.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand the organization’s performance improvement methodology and contribute to quality service provision.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months of outpatient clinical experience.
• CPR certification.
• Active and applicable license for the state where work will be performed.

Schedule

This position requires a 5x8.0 hour shift schedule.
