Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to patients' needs, goals, and physician direction. This role involves delegating and directing discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate a successful return to community activities. The therapist also serves as an educator for patients, families, students, and coworkers while upholding professional standards and organizational compliance.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document all patient interactions and progress in a clear and timely manner.
• Ensure care is provided under signed, valid physician orders and insurance approvals.
• Participate in the student program by providing supervision and evaluations.
• Create an individualized professional development plan with clinical management.
• Adhere to discipline-specific State Practice Act and maintain licensure in good standing.
• Engage with performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active applicable state licensure for the practice of Physical Therapy.
• Previous experience with the agency is highly desired.
