Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician directives. This role includes the responsibility of delegating and directing all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that facilitate effective returns to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves, while performing other professional duties related to patient care and the development of discipline-specific therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge.
• Document medical records clearly, concisely, and accurately in a timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student program assignments, providing positive learning environments and evaluations.
• Create an individualized professional development plan with clinical management and pursue professional growth.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and engage in the organization’s performance improvement methodologies.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active applicable license for the state where work will be performed.

Schedule

This is a 13-week contract position with a 5x8.0 schedule.
