Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patient’s needs, goals, and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring effective return to community activities through functional treatment programs. The Physical Therapist also acts as an educator for patients, families, students, coworkers, and themselves.
Responsibilities
- Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans.
- Document medical records in a clear, accurate, and timely manner, ensuring compliance with federal and state guidelines.
- Ensure care is provided under signed, valid physician orders within necessary insurance approvals.
- Participate in student programs, providing a positive learning environment with supervision and evaluations.
- Create and execute an individualized professional development plan to pursue ongoing education and growth.
- Adhere to the State Practice Act and maintain professional licensure in good standing.
- Demonstrate awareness of organizational performance improvement methodologies.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- Minimum of 1 year of outpatient clinical experience.
- Current CPR certification.
- Active licensure as a Physical Therapist in the state where work will be performed.
Schedule
This position is structured as a 5x8.0 shift schedule.
Assignment Duration
This is a 13-week contract position.
