5x8.0 schedule from 7:00 AM to 3:30 PM daily.

Job Description

The Outpatient Physical Therapist evaluates assigned patients, developing, implementing, and modifying treatment plans based on individual needs, goals, and physician direction. Responsibilities include delegating and directing therapy services, ensuring the design of effective treatment programs, and serving as an educator to patients, families, students, and colleagues. The therapist will uphold professional standards, demonstrating behaviors consistent with the organization's mission and core values, while also ensuring compliance with safety policies and procedures.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and create individualized treatment plans.
• Document care accurately and timely, ensuring compliance with state and federal guidelines.
• Participate in student programs by providing supervision, developing learning objectives, and completing evaluations.
• Engage in continued professional development through education and professional organizations.
• Adhere to ethical standards and maintain professional licensure in good standing.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for practicing Physical Therapy.
