Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients. The therapist ensures the design of functional treatment programs that facilitate an effective return to community activities and serves as an educator for patients, families, students, and coworkers.

Responsibilities

• Completes comprehensive patient evaluations and develops appropriate treatment plans and goals.
• Documents medical records clearly, concisely, and accurately in compliance with state and federal guidelines.
• Conducts therapy under signed physician orders and within insurance approval as necessary.
• Participates in student programs, providing supervision and evaluations.
• Implements a professional development plan, pursuing continuing education and professional activities.
• Maintains professional licensure and adheres to discipline-specific codes of conduct and state practice acts.
• Understands and actively participates in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for practice in the location assigned.

Schedule

This position follows a 5x8.0 schedule.

Assignment Duration

This is a 13-week contract position.
