Licensed Practical Nurse (LPN) - Full Time, Days and Nights

Schedule

This position requires a full-time schedule encompassing both day and night shifts, working five 8-hour shifts per week, including every other weekend and holiday.

Job Description

The LPN is responsible for providing safe and effective nursing care to individuals and groups of residents, demonstrating appropriate knowledge and skills for patients of varying ages. The role emphasizes collaboration with nursing staff, physicians, and ancillary staff to ensure comprehensive resident care.

Responsibilities

• Collect and communicate data relevant to residents' physical and psychosocial conditions to RNs and the interdisciplinary team.
• Contribute to the development of a culturally relevant plan of care that reflects nursing diagnoses and interventions.
• Evaluate and amend plans of care based on resident outcomes and suggest referrals as necessary.
• Mentor and orient new staff while maintaining clinical competency through ongoing education and in-service programs.
• Exhibit leadership and collaboration under the direction of RNs, ensuring patient and family engagement in care processes.
• Address safety, cost, and quality considerations in care planning and delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI specifically)
• A minimum of 1 year of experience as an LPN in a Long-Term Care (LTC) facility
