Physical Therapist

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs and physician direction. The role includes the delegation and direction of all discipline-specific therapy services for those assigned patients. The therapist ensures the design of functional treatment programs that allow for effective return to community activities, serving as an educator for patients, families, students, coworkers, and self. The position also involves performing other professional duties associated with patient care and the development of discipline-specific therapy services.

Responsibilities

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans focused on progressing patients towards discharge.
• Collaborates with interdisciplinary teams to address all discharge barriers, ensuring timely trials and orders of necessary equipment.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Documents medical records accurately and timely, ensuring compliance with policies and state regulations.
• Participates in the training and supervision of students and contributes to a positive learning environment.
• Actively engages in professional development and advocacy efforts within the community and professional organizations.
• Maintains professional licensure and adheres to ethical and regulatory standards, contributing to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• BLS Certification required prior to the first shift.
• Valid and current State license as a Physical Therapist.
• Minimum of 2 years of experience in a relevant setting.
• Previous experience in Long-Term Acute Care Hospitals (LTACH) is preferred.

Schedule

This is a Day shift position, with a 5x8.0 hours schedule.
