Day shift, 5x8-Hour (08:00 - 16:30), no weekends required.

Job Description

We are seeking a dedicated Home Health Physical Therapist to provide exceptional care to patients in their homes. The successful candidate will have at least one year of skilled home health experience and will work one-on-one with patients, visiting up to six patients a day. This position requires the ability to navigate effectively between appointments while ensuring high-quality care and compliance with documentation standards.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for home health patients.
• Administer physical therapy interventions following established guidelines and best practices.
• Ensure accurate documentation in EPIC/Oasis system.
• Coordinate care with interdisciplinary team members as needed.
• Educate patients and their families on rehabilitation techniques and home exercise programs.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• BCLS Certification required.
• Oasis experience required.
• Must possess reliable transportation for patient visits.
• Primary series COVID vaccination, including one booster required.
