Day shift, 5x8-Hour (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Physical Therapist to provide individual therapy services in home health settings. The Physical Therapist will be responsible for evaluating patients, developing treatment plans, and delivering effective therapy interventions tailored to each patient's needs.

Responsibilities

• Conduct patient evaluations and assessments in home health environments.
• Develop and implement individualized treatment plans based on patient needs.
• Monitor and document patient progress and modify treatment plans as necessary.
• Educate patients and families on treatment options and home exercises.
• Manage a caseload of up to 6 patients per day, ensuring adherence to productivity points standards.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• BCLS certification required.
• Experience with Oasis is mandatory.
• Reliable transportation is essential as travel may be required.
