Schedule

Day Shift: 5x8-Hour (08:00 - 16:30) with weekend coverage required (at least 2 weekends in a 4-week schedule).

Job Description

The Acute Care Case Manager will be responsible for comprehensive care coordination and discharge planning for adult and geriatric patients. The ideal candidate will ensure that each patient's plan of care aligns with their needs, facilitating approved authorizations and utilizing InterQual criteria effectively.

Responsibilities

• Conduct patient assessments and care coordination activities.
• Utilize InterQual criteria for admission and benefits eligibility.
• Oversee discharge planning and ensure smooth transitions of care.
• Collaborate with interdisciplinary teams, including social services.
• Document appropriately using Epic charting system.

Required Experience / Certifications / Licensure

• Minimum of 2 years of current experience in acute hospital case management.
• Valid California state license required; pending licenses accepted.
• Experience with Interqual and Epic charting systems.
• Must have supervisory references from prior employment.
