Case Manager

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30); requires working 2 weekends every 6 weeks.

Job Description

The Case Manager will be responsible for providing comprehensive care coordination, ensuring that patients receive the appropriate evaluations and interventions during their stay. The successful candidate will work closely with interdisciplinary teams to develop and implement care plans tailored to individual patient needs.

Responsibilities

• Conduct thorough assessments to determine patient care needs.
• Develop and coordinate treatment plans in collaboration with healthcare teams.
• Facilitate family meetings and assist in organizing necessary resources such as DME, SNF, and ambulance transport.
• Ensure compliance with CMS, HIPAA, and Joint Commission standards.
• Manage case loads effectively, adhering to patient ratio requirements.

Required Experience / Certifications / Licensure

• Minimum of 2 years of case management experience in an acute hospital setting.
• Experience with Epic charting system is required.
• Experience in LTAC and Trauma Level I or II is preferred.
• Current BLS certification required.
• Must float between like units as needed.
