Acute Care Case Manager

Day Shift: 5x8-Hours (08:00 - 16:30), with every other weekend required.

Job Description

The Acute Care Case Manager will handle care coordination, discharge planning, and utilization review in an acute care setting. The ideal candidate will have a demonstrated background in case management, particularly within California, and possess knowledge of InterQual criteria and Epic charting systems.

Responsibilities

• Conduct assessments to determine patients' needs for care coordination.
• Perform discharge planning and ensure a smooth transition for patients.
• Utilize InterQual criteria for concurrent and retrospective reviews.
• Work collaboratively with interdisciplinary teams to enhance patient care.
• Maintain compliance with regulatory agencies and organizational standards.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in acute care case management, specifically in California.
• Experience with charting systems, particularly Epic.
• Strong knowledge of care coordination, utilization review, and discharge planning.
• Must possess a valid California state license.
