Day shift, 5x8-hour schedule (08:00 - 16:30). Must be available for every other weekend and holidays.

Job Description

We are seeking an experienced Acute Care Case Manager to join our team. The ideal candidate will be responsible for case management and discharge planning in an acute care facility, working within a collaborative interdisciplinary team. You will handle a case load of up to 25 patients, coordinating care, and ensuring smooth transitions of care.

Responsibilities

• Conduct assessments and develop individualized treatment plans.
• Coordinate and oversee discharge planning processes.
• Ensure that care meets medical necessity and is compliant with applicable guidelines.
• Participate in family meetings to discuss care and treatment options.
• Collaborate with interdisciplinary teams including social services, pharmacy, and physical therapy.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care case management experience.
• Current California state licensure required; pending licensure accepted if obtained by start date.
• Experience with InterQual criteria and guidelines is preferred.
• Knowledge of Medicare, Medi-Cal, and insurance processes required.
