LPN Full Time Evening Shift

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide care needed by adult and geriatric patients.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of care reflecting nursing diagnosis and interventions.
• Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
• Maintain current knowledge and competency in Nursing Practice and participate in the mentoring/orientation of staff.
• Under the direction of the RN, provide leadership in the professional practice setting.

Required Experience / Certifications / Licensure

• NY LPN Licensure
• BLS (AHA or RQI only)
• 1 year of experience as an LPN

Schedule

This position requires full-time evening shifts, working 5x8 hours with D/E/N/W rotations and holiday requirements.
