Part-Time LPN (Evenings) - New York

Part-time evenings (2 shifts in week 1, 3 shifts in week 2), with weekend and holiday shifts required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents while demonstrating knowledge of the age-specific needs of adult and geriatric patients. Under the supervision of a Registered Nurse, the LPN is responsible for maintaining a collaborative relationship with other nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect continuous data regarding the residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and interventions.
• Evaluate resident outcomes and recommend revisions to care plans as necessary.
• Maintain current clinical knowledge and competency through ongoing education and participation in mentorship.
• Demonstrate leadership within the nursing practice setting while considering safety and cost factors in care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification through AHA or RQI.
• Minimum of 1 year of experience serving in an LPN capacity at a long-term care facility.
